Manage the SOAP Process and Templates
Subjective
ThoroVet allows you to do SOAP based evaluations. You can customize components of these evaluations to suit the needs of your medical records and evaluations.
Managing the Subjective portion of the evaluations can be done from the Manage Account section. To access click on the Manage account button on the left-hand toolbar or from the main menu. Once in the Manage account section click on Manage SOAP.
There are two main components to the Subjective section. The Subjective Location and Description. The Location allows you to pick an area of concern that the Evaluation may be concentrating on. The Description allows you to describe what the concern relating to the location may be.
Subjective Location – You can Add/Remove/Edit the Subjective locations as needed.
- To add a new Location, click on the Add Subjective Location button. This will open a new row and allow you to type in a location.
- To Edit Location – Click on the Edit button and this will allow you to edit the existing location.
- Delete – Click on the delete button and this will remove that subjective location from future evaluations
- Import/Export – You can also import a bulk set of subjective locations. To do so click on the Import/Export button. From here you can export the current list of locations, download a template to use and import a completed spreadsheet.
Subjective Descriptions - You Can Add/Remove/Edit the Subjective descriptions as needed.
- To add a new description, click on the Add Subjective description button. This will open a new row and allow you to type in a description.
- To Edit Description – Click on the Edit button and this will allow you to edit the existing description.
- Delete Description – Click on the delete button and this will remove that subjective description from future evaluations.
- Import/Export – You can also import a bulk set of subjective descriptions. To do so click on the Import/Export button. From here you can export the current list of descriptions, download a template to use and import a completed spreadsheet.
Objective
ThoroVet allows you to do SOAP based evaluations. You can customize the components of these evaluations to suit the needs of your medical records and evaluations.
ThoroVet will come pre-loaded with a general Objective template. You can customize this standard template or create new specific templates. The objective template selected when starting the Evaluation will determine the type of physical exam recordings you can make.
Managing the Objective portion of the evaluations can be done from the Manage Account section. To access click on the Manage account button on the left-hand toolbar or from the main menu. Once in the Manage account section click on Manage SOAP.
Adding New Template – If you wish to Create a New Template, click Add New Objective Template Button. This will open a new Objective Template screen.
- Template Name - The Name of the Exam Template
- Group – Can create groups of recordings, which can be used on other Templates.
- Items – There are 8 different types of recordings that can be made. The type of item you utilize will determine the behavior when interacting with it during the Evaluations.
- Text Box – This field will give you a free text box to type into
- Label – Name of the recording
- NSF – No
- AutoComplete – Will autocomplete based on previous recordings.
- Default Value – A Default recording for this field
- Slider – This will present as a numeric slider. Example would be for use with temperatures. Can have Min/Max and Default value associated to the slider to control the range
- Label – Name of the field
- NSF – No
- Min Value – The starting point of the slider scale
- Max Value – The Ending point of the slider scale
- Default Value – The default and starting point
- Switch – This will present as a true/false or Yes/No type of a recording.
- Label – Name of the Recording
- NSF – No
- Default Value – Either Yes or No
- Drop Down – Is a drop-down selection, only one selection allowed
- Label – Name of Recording
- NSF – No
- Options – All the possible selections in the drop-down menu
- Default Value – The Default recording for this drop down
- Combo Box – Very similar to Drop Down Menu but will allow you to type selections in and those will be available on future evaluations.
- Label – Name of Recording
- NSF – No
- Options – All the possible selections
- Default Value – The Default selection
- Radio Button – These will present as a group of options and only allow one selection.
- Label – Name of the recording
- NSF – No
- Options – All of the possible selections
- Default Value – The pre-selected/default values
- Check Box – Similar to the Radio button, but this one will allow multiple selections
- Label – Name of the Recording
- NSF – No
- Options – All of the possible Selections
- Default Value – The pre-selected/default values
- Numeric Text Box – This will present as a number box.
- Label – Name of the Recording
- Decimal Value – Defines how many decimals the numeric value will accept
- Default Value – The default value for this recording
- Text Box – This field will give you a free text box to type into
- To Add an Item to the Template, Drag and drop it into the corresponding group you want the recording to be a part of.
- To Delete an Item – click the Red X next to the recording you wish to delete.
- To Reorder – click the three dashes and then drag and drop the items into the desired order.
- Click Save.
Edit Template – If you wish to edit an existing template, you can add or remove items as well as change defaults or options within the recordings.
- Click Edit button on the Template you wish to edit
- Once the Template loads you will see all the recordings associated with this template.
- To Edit a recording click the Pencil button next to the item you wish to edit. This will bring up the item properties screen where you can make the appropriate changes. See the Items section under Adding New template for definitions of each option.
- To Delete a Recording – Click the Red X on the row of the item you wish to delete
- To Re-Order – Click the three dashes button, which will then allow you to drag and drop rows into the proper order.
Assessment
ThoroVet allows you to do SOAP based evaluations. You can customize the components of these evaluations to suit the needs of your medical records and evaluations.
Managing the assessment portion of the evaluations can be done from the Manage Account section. To access it click on the Manage account button on the left-hand toolbar or on the main menu button. Once in the Manage account section click on Manage SOAP and then Assessment.
Add New Assessment – You can add new assessments to be available from your evaluations. To do so you can click on the Add Assessment button. This will open a new screen where you can fill in the required information.
- Name – The Title/Name of the Assessment
- Category – The type or category of the assessment
- Description – Can add additional notes relating to this assessment.
- Discharge Summary Notes – Here you can add notes that will show up on the discharge charge summary based on the selected assessments.
Edit Assessment – Click on the row of the assessment you wish to edit. This will bring up the same adding new Assessment screen shown above. Once opened click the pencil icon in the upper right-hand corner which will open the form for editing.
Delete Assessment – Click the delete button next to the row you wish to delete. This will remove the assessment option from future evaluations.
Import/Export – You can bulk import/export the assessments using the Import/Export button.
